Admission to Garden Manor is as easy as 1-2-3! After you have decided the Garden Manor is the best choice for you or your loved one, all we need to do is the following:
Collection of Required Information:
Completion of a Residency Application and Security Deposit will be needed to hold the apartment of your choice. We will need copies of Medicare and Insurance Cards. If the following documents are in place, we’ll need copies of Power of Attorney, Durable Power of Attorney for Healthcare and /or Living Will.
Medical History:
Your physician will need to complete a Medical History and Physical form and return it to the facility.
Assessment:
Our staff will schedule and complete a pre-admission Assessment to determine the resident’s needs and level of care upon admission. Upon completion of this process, a Room Agreement will be signed and an admission date will be set.
